Our Care Managers are Registered Nurses and Licensed Social Workers who specialize in working with seniors and their families to coordinate their care needs. Services include: Assess current needs and strengths and devise a plan of care; research and arrangement of resources; medical advocate at appointments, hospital stays, care conferences etc.; coordination of communications among providers and family; caregiver support; assist with relocation locally and out of state; “eyes and ears” for family members that cannot be present; moderate family meetings & much more.
