Alliance for Integrated Medication Management AIMM is a non-profit organization working to support widespread adoption of team-based medication management services into the care of high-risk patients.

The Alliance for Integrated Medication Management (AIMM) is a non-profit organization working to support wide-spread adoption of team-based medication management services into the care of high-risk, high-cost patients suffering from multiple chronic health conditions. AIMM fosters community partnerships to establish health care delivery systems specifically for patients with multiple chronic conditions who need help with adherence and monitoring of their medication regimens. Teams of health care professionals integrate clinical pharmacy services with primary care services in order to best support patients and providers with coordinating medication use and achieving desired clinical parameters, such as lower blood pressures and target A1C levels. Are you providing comprehensive medication management services? The is creating the National Registry of Comprehensive Medication Management Practices©, so take a few minutes to offer information on your program.

The GTMRx Evidence-Based Resources Subgroup is working to identify and showcase those who are practicing CMM across the country in order to better characterize where CMM is occurring and provide better resources for emerging and existing CMM practices, which will lead to better access to CMM services and help to achieve practice, payment and policy reform.

We celebrate our recent A3 Collaborative cohort, made up of pharmacy-led interdisciplinary health care teams, who demonstrated remarkable leadership in a time of crisis and found opportunities to continue and enhance patient care!

Celebrating the success of the A3 Collaborative and preparing for the next evolution of CMM!

AIMM will not immediately resume another A3 Collaborative offering this fall. Going forward, AIMM and partners will work to redesign the collaborative to address the technical and policy challenges of achieving full scale CMM, particularly in light of the evolving healthcare transformation due to COVID-19.

We envision an A3 Collaborative that can help communities:
- Identify and engage populations who require CMM for successful health outcomes.
- Establish CMM operations at scale, across health care systems, regionally, and statewide.
- Generate innovative value-based payment arrangements crafted regionally and locally to finance these service delivery models.
- Continue to evolve CMM operations as part of integrated delivery systems.

A3 Collaborative Teams embrace innovation to meet new demands.

A large health system was experiencing rapid growth and pharmacists across the system were concerned that the care process for comprehensive medication management (CMM) services would become inconsistent across practices and produce variable outcomes and value. Pharmacy care teams across the system united in agreement to implement CMM services using a consistent care process; one that ensures fidelity.

High demand for CMM services in a rural health clinic practice made it a challenge for the pharmacy team to meet the need with limited resources and personnel. The clinic partnered with a local college of pharmacy school to establish an offsite patient call center to triage medication-related issues and patient resources.

Pharmacists in a practice clinic supplied patients with personal Continuous Glucose Monitoring devices to track patients’ clinical data remotely and for over a period of time. This device prevented the need for patients to make frequent visits to the practice, which is desired during the pandemic, while still allowing clinical data to be collected and reviewed in a timely manner.

Innovations like these led the A3 Collaborative participants to collectively brainstorm and then identify six drivers to foster innovation for CMM services. These drivers for innovation can be applied across all levels of health care delivery.

Transition-of-Care Pharmacists Fill Gaps During COVID-19

AIMM's A3 Collaborative organizations are profiled for their ability to close the gaps in care during COVID-19. A3 Collaborative project lead, Toni Fera, emphasizes: Pharmacists are learning about COVID-19 with the rest of the system. They’re right on the front lines and they know what’s going on. They’re filling in the gaps in knowledge for patients and physicians.

AIMM has been facilitating special COVID-19 virtual events since March, 2020, for its A3 Collaborative teams to share challenges they are facing as they navigate the demands of this crisis, within their medication management services. These webinars have focused on redesigning delivery systems for new demands, offering telehealth services, and addressing rapidly changing federal and state policies.

Recently, the pharmacy team led by Michael Korczynski, PharmD, at Allegheny Health Network (AHN) in Pittsburgh, reached out to a healthy 22-year-old patient.

Using Implementation Science in the initiative to catalyze change in community .

The Slice of PIE initiative is a collaborative effort between HealthPartners (HP), the University of Minnesota (UMN),the University of North Carolina (UNC), and the Alliance for Integrated Medication Management (AIMM) to improve and increase delivery of medication therapy management (MTM) within the HealthPartners MTM provider network. It takes its name from HealthPartners’ “Partners in Excellence” (PIE) program, a health plan-wide program to recognize and reward outstanding contributions to beneficiary health. The Slice of PIE initiative is driving impact on care delivery by merging a facilitated community of learning structure around a performance-based payment model for MTM services.
