๐We are buzzing with excitement at Gracepoint! ๐
What started as an idea for a small gathering turned into something far more impactful than we ever expected.
Yesterday we hosted the inaugural Gracepoint Care Summit, bringing together leaders from across the care continuum - including Home Care, Home Health, Hospice, Primary Care, Assisted Living, LTC, Care Advisors, Memory Care and more. What quickly became clear is just how much we all need spaces like this to openly share challenges, ideas, and solutions.
The conversations were honest, insightful, and incredibly energizing. When professionals who all care deeply about serving seniors and their families come together, the possibilities for improving care grow exponentially.
When we break down silos and work together, the real winners are the patients and families we serve.
