English Oaks Convalescent & Rehabilitation Hospital

English Oaks Convalescent & Rehabilitation Hospital English Oaks is a 180-bed skilled nursing facility. English Oaks Nursing & Rehabilitation focuses primarily on post-hospitalization transitional care.

Our greatest success is the ability to help patients recover and discharge back to home or to a lower level of care! We employ our own therapists and operate a large, fully contained therapy unit. Together, with our nursing staff, our goal is to help patients achieve their highest level of functional independence. View a virtual tour of our facility here:

At English Oaks Nursing & Rehabilitation in Modesto, CA, Linda S. arrived after a ground-level fall, feeling fearful and needing full assistance with mobility and daily tasks.

With steady encouragement, hands-on therapy, and the consistent support of our physical and occupational therapy teams, Linda rebuilt her confidence step by step. Today, she’s walking with a front-wheeled walker, regaining independence in her daily activities, and preparing to transition home safely.

Progress is powerful—and Linda’s determination made it possible. 💙
