Nutrition Healthy Choice announces it sponsorship of the Child and Adult Care Food Program. All children in
attendance will be offered the same meals at no separate charge and with no physical segregation of or other
discrimina tion against any child because of race, color, national origin, s*x, disability, age, or reprisal or retaliation
for prior civil rights activity in any program or activity conducted or funded by USDA.
Persons with disabilities who require alternative means of communication for program information ( e.g.
Braille, large print, audiotape, and American Sign Language), should contact the responsible State or local
Agency that administers the program or USDA's TARGET Center at (202) 720-2600 (voice and TTY) or contact
USDA through the Federal Relay Service at (800) 877-8339.
To file a program discrimination complaint, a complainant should complete a Form AD-3027, the USDA
Program Discrimination Complaint Form, which can be obtained online,
https://www.ocio.usda.gov/document/ad-3027, from any USDA office, by calling (866) 632-9992, or by writting
a letter addressed to USDA. The letter must contain the complainant's name, address, telephone number, and a
written description of the alleged discriminatory action in sufficient detail to inform the Assistant Secretary for
Civil Rights (ASCR) about the nature and date of an alledged civil rights violation. The completed AD-3027 form
or letter must be submitted to USDA by:

POSITION OPEN-------Shreveport/Bossier City Office

Position Title Monitor (office and field)

Education, Minimum Training and Experience Required: Experience in Social Work or related field expertise; Ability to follow program guidelines. The desired candidate must have strong interpersonal and communication skills. The ability to handle constant and recurring deadlines. Must have the ability to think critically and make decisions independently.

Job Description: Monitor providers in their homes to assure they adhere to FDCHFP guidelines. Do pre-approval review of provider’s homes prior to the State Fire Marshal inspection. Reports to the program manager.

Job Responsibilities: The successful candidate will be expected to:
-verifying the information on the enrollment form for each child.
-assisting with pre-approvals and mail-out re-certifications letters
-monitoring providers in as assigned
-train new providers before they are submit the first menu for reimbursement
-randomly calling providers during meal times
-attends monthly meetings and provider in-service designed to maintain proficiency and/or increase knowledge of the FDCHFP.
-accepts and performs additional duties not listed in the job description but necessary for the smooth operation of NHC.
-assist with answering telephone, filing, and the day-to-day operation
-responsible for recording, each month, on the daily attendance sheets, date, child(ren)’s name, age, meal types, and holidays.
-prepare menus packets for each provider monthly.
-check of ‘who keeps’ the children of providers when they are sick.
-review monitoring paperwork to assure monitoring forms, meals
monitored, time sheets and travel expenses are consistent and
accurate.
Other duties as assigned.

Special Instructions to Applicants Reliable transportation, cleared driving record, and insurance is required
Job will require approximately 35% office work and 65% field travel.
