Standing Stone Care & Rehabilitation Center Signature HealthCARE is an award-winning, family-based organization revolutionizing healthcare. We believe in living life with purpose and serving others.

Located right off I-40 in the small town of Monterey, Tennessee, Standing Stone Care & Rehabilitation Center is a 98-bed skilled nursing facility that has both short-term and long-term care programs and services to provide our residents with quality health care. We have 18 private rooms that are designed for the skilled rehabilitation services. The rooms are secluded from the long-term section of the building. These rooms offer a home-like atmosphere that features a 32-inch flat screen television. Patients and residents will also enjoy the comfort of an electric bed. This wonderful experience will surely be a breath of fresh air during a very difficult time. All programs are comprehensive and can be specialized to fit the requirements for each resident or patient. With the convenience of three hospitals located in Cookeville, Crossville, and Livingston, all within a 20-mile radius, Standing Stone provides a home-like environment with a compassionate and proficient staff that looks after the needs of the residents and patients.

Standing Stone Care and Rehab has found our new Director of Nursing! She is a hidden gem that has a heart of a true leader and is ready to help make us shiny again! We are so excited to welcome Carin Bray! She has been in Long Term Care for...........she just says, "a long time"! She is a big Vols football fan, loves true crime documentaries, and doesn't like germs, onions, or laziness! She has a son in college at Tech, and a daughter in middle school and has been married for 23 years! Carin, pronounced as "CA-rin", is excited to be joining the Standing Stone team!

Celebrating Debbie Street – 35 Years of Dedication and Heart

After 35 wonderful years with Signature HealthCARE, Debbie Street, CEO of Signature HealthCARE of Elizabethton Rehab & Wellness Center is retiring.

Debbie began her journey with Signature in the Business Office and Human Resources, and over the years, her leadership and compassion have left an enduring impact on residents, staff, and the community she calls home.

“What I love most about Signature is our focus on residents’ happiness and staff engagement,” Debbie shares. “I’m incredibly grateful for the opportunities for growth and learning that Signature HealthCARE has provided me. I’ve had the chance to collaborate with remarkable colleagues and be part of many exciting projects that have truly changed the landscape of long-term care.”

Having grown up in the Elizabethton community, Debbie has known many residents and their families personally. She treasures the moments spent listening to their life stories — moments that reflect the deep sense of family and connection that define Signature’s mission.

As she steps into retirement, Debbie looks forward to spending more quality time with her family, especially serving as her granddaughter’s chauffeur to school and sports. She also plans to indulge in her love for DIY projects, church work, and traveling — taking time to simply slow down and enjoy life’s blessings.

“I sincerely appreciate the relationships I’ve built and the experiences that have enriched my life,” Debbie says. “Thank you for the opportunities and trust you’ve placed in me during my 35 years. I wish Signature HealthCARE continued success. God bless.”
