🛑 Stop using a Ferrari to deliver DoorDash!!!

Physical therapists keep asking why reimbursement feels broken…

while using doctoral-level talent on tasks that don’t require doctoral-level skill.

You have a doctorate.

Yet some clinics are paying premium clinical labor to do chunks of the day that could be handled differently, delegated appropriately, or systemized better.

😳 Then everyone acts shocked like it’s a Netflix documentary.

“Where did the money go?” 🤷‍♂️

Probably into using premium labor for basic tasks.

Imagine hiring a heart surgeon to check people in at the front desk.

Imagine paying a pilot to hand out peanuts.

Imagine calling LeBron in to organize the locker room towels.

Ridiculous, right?

Then why do we normalize it in rehab?

And right on cue comes the comment:

“But only I can deliver the care MY patient needs one-on-one for a full hour.”

Cool. Then sell that.

Seriously. Sell THAT.

If your model is true high-touch, one-on-one, premium care… charge a premium rate to patients who value that premium experience.

That’s called positioning.

But if you signed an insurance contract paying X dollars for that visit… then that’s the deal.

Hard stop.

You can’t sell steak at hamburger prices and then scream at the cow. (Write that down)

Insurance is buying efficiency, standardization, and access.

Patients paying cash are buying time, attention, customization, and exclusivity.

Those are different products.

Neither is evil.

But pretending they should pay the same is where the delusion starts.

The highest trained person in the building should spend most of their time doing the highest-value things only they can do:

- evaluation
- decision-making
- solving complexity
- progressing care
- behavior change
- outcomes

Build systems. Build lanes. Build options.

Want higher pay?

Either create leverage…
Or create luxury.

But stop using a Ferrari to deliver DoorDash…
