Today we had the honor of attending the funeral service of Gloria L. Gonzalez 🤍🕊️ A beautiful soul whose life touched so many hearts. It was a meaningful and emotional gathering filled with love, memories, and the reminder of the lasting impact one person can have on generations 🌹✨

Our very own Sharon Hammond, RN, and Chaplain Rebecca Tong were present to support Gloria’s family, offering comfort, compassion, and continued care during this important time of remembrance 🤍🙏 Their presence reflects the heart of hospice — walking beside families not only in life, but in honoring their loved one’s legacy 🕯️

At Gentle Transitions Hospice, we believe our care does not end at the bedside. We remain a source of support, comfort, and compassion for families every step of the journey 🕊️🤍

Love was definitely in the air at Arlington Residence and Rehabilitation Center! ❤️👑

Our Gentle Transitions Hospice team had the sweetest time celebrating Valentine’s with these amazing residents. From crowning royalty to sharing laughs, pizza, and smiles, it was a beautiful reminder that love, dignity, and joy never fade.

Bill Storey, Gwenn Dalton, and Rosie Chapman from Here at Home helped make the day extra special—bringing compassion, connection, and a whole lot of heart to everyone in the room. These moments are why we do what we do.

Kathy Stephens worked tirelessly to reunite one of our patients with their grandchildren from Peru — all so this family could have the chance to say a proper goodbye.

Kathy has been a guiding light — providing comfort, strength, and unwavering support to our hospice families during some of life’s most tender moments. Her ability to truly listen, her authentic care for every individual she’s served, and her commitment to walking alongside families through their journeys is nothing short of inspiring. 🌷

From sharing smiles and treats with residents alongside our team to being a steady source of comfort, Kathy’s presence has made Gentle Transitions Hospice a kinder, warmer place for all who have had the privilege to know her. Her impact will be felt for years to come.

About Us

Gentle Transitions Hospice is a locally and family owned hospice agency in Central Texas. We provide quality end-of-life care for patients and their families. We service a 60-mile radius of Waco-Temple-Killeen, Texas. We provide skilled nursing care, physician oversight, social workers, chaplains, certified nursing assistants, and volunteers for our hospice patients. We are proud to serve our local Veterans as well, as we are contracted with the Department of Veterans Affairs. We are also ACHC Accredited and Medicare Certified.

We Believe in Old-Fashioned Hospice

Gentle Transitions Hospice is a locally and family owned business. The owners are mother and son, Gwenn Dalton and Evan Hessel. Gwenn has been a Hospice Registered Nurse, Director of Nursing, and Administrator in Central Texas for 20+ years and has a true passion for End-of-life Care. Evan has worked in hospice marketing and administration for the past 10 years, though he will always tell you that he grew up in hospice, with a deep respect for it. Gwenn and Evan dreamt that one day they might have their own "Old-fashioned" hospice. What they meant by that is, after both having worked for large hospice companies and corporate entities, they wanted to return to a time when patient care was the #1 priority, as opposed to only being concerned with the bottom line. Gwenn recalls when she first started hospice nursing, saying, "The hours were long and the work was hard, but at the end of the day our patients and staff always felt greatly appreciated." And this is the exact environment that will be encouraged at Gentle Transitions Hospice. We want to treat our staff with love and care, so they pass on that same blessing to every single one of our patients and their families. At Gentle Transitions Hospice everyone is family!
