Ketamine works differently from other antidepressants. The prevailing theory is that it affects the brain’s glutamate system, which scientists now realize may be involved in depression, rather than the better-known serotonin pathway used by drugs like Prozac. Animal research suggests that partly blocking certain glutamate receptors increases brain plasticity — the ability of the brain to make new neuronal connections — and corrects some of the abnormalities that result from chronic stress. These salutary effects on the brain, coupled with how quickly ketamine works, have inspired a flurry of research. A number of drugs either derived from ketamine, or based on how scientists think it works, are in development. The pharmaceutical company Janssen is working on a nasal spray.

It’s been way too long since there was a new class of drugs to treat depression. Ketamine might be the solution.

For the first time since 1977, the FDA has updated guidance on the development of medications to treat major depressive disorder. There has been a huge shift in direction and it all started with Ketamine; an anesthetic accidentally found to be an anti-inflammatory, BDNF increasing, monoamine neurotransmitter receptor modulating, immune system regulating, NMDA, GABA, CAMP and Op**te receptor acting wonder drug. There will never be another SSRI, SNRI or tricyclic antidepressant brought to market. That is all "old" science. So excited to see what the future will bring for our treatment resistant patients.

In an article published recently in the journal Cell Reports, David Olson, Calvin Ly, and colleagues investigate the biologic actions of a class of drugsthat they have named psychoplastogens. The authors provide the following description of drugs that fall into this new class: “To classify the growing number of compounds capable of rapidly promoting induced plasticity, we introduce the term ‘psychoplastogen’ from the Greek roots psych- (mind), -plast (molded), and -gen (producing).” These drugs cause nerve cells in the brain to form new neurites, i.e., projections that extend out from the cell body and have the potential to become axons and dendrites. In addition, these agents enhance the ability of nerve cells to interact with other nerve cells by increasing the number of synapses — the regions where nerve cells connect with one another.

Psychoplastogens enhance connections between brain cells.
