About Our Practitioner's:

Chris Wade is a Physician Assistant, PA-C, who graduated from Augustana College with a bachelor's degree in Liberal Arts. He then attended Midwestern University in Chicago where he obtained his PA degree. He has 15 years of emergency medicine experience and also has experience in urology, family medicine and in geriatrics. he developed an interest in anti-aging-functional medicine after working in the other areas of medicine where chronic diseases have become common practice. Chris loves being able to prevent chronic illness and promote health by practicing functional medicine. He firmly believes that chronic illness can be reversed in many cases. He studied this specialized medicine and became certified through Worldlink. He is very excited to help patients become healthy and to assist them in maintaining that health. He looks forward to working with you to design the perfect health plan to meet your goals. Chris enjoys working out, traveling and spending time with his family and friends. He lives in Washington, IL with his wife Jen and his daughter Madison. Elizabeth Hayes is an Advanced Practice Nurse, APN, who received a bachelor's degree from OSF St. She attended Loyola University in Chicago and was enrolled in the emergency nurse practitioner program. She also attended and earned her master's degree from OSF St. Francis Medical Center with a practitioner degree in acute care adult. She went on to get a post master's degree as a practitioner in acute care pediatrics. Beth has worked in emergency medicine for many years which is her first love. She became intrigued by anti-aging-functional medicine a few years ago after being exposed to it by another provider who was working in both emergency medicine and functional medicine. She studied this specialty through the American Academy of Anti-Aging medicine, A4M. Functional medicine became her passion as she watched her patients experience a new life by optimizing what their body was originally designed to do. She believes that everyone has their own specific optimal level that needs to be obtained to feel their very best. Beth is excited to work with you and lead you to a better quality of life. She enjoys sports, everything outdoors and has a passion for animals. She lives in Morton, IL and loves spending time with her daughters Abbie and Ellee.
