We are proud to celebrate the incredible Social Workers who serve our residents, patients, and families every single day. In long-term care, Social Workers are advocates, educators, and problem-solvers. They protect resident rights, guide families through complex decisions, and help create stability during some of life’s most vulnerable moments.

We are especially proud to recognize Santana Polk, whose compassion, professionalism, and dedication make a meaningful difference in the lives of those we serve. She is a valued asset to our team and a true example of what Social Work leadership looks like in action.
