Brief history:
📌Chronic UTIs for many years, though some cultures came back negative for infection
📌Chronic constipation
📌Recent cystoscopy (all clear) & kidney ultrasound for kidney stones (negative) by her urologist
📌Urologist prescribed daily low dose antibiotics to prevent future UTI and a urinary analgesic medication to take with flare ups of bladder pressure and urgency
➡️She did not want to be on these medications indefinitely and sought out pelvic floor therapy
🗓️Patient came in for her pelvic floor therapy initial evaluation in November and has had 5 follow up sessions since
📈Most significant findings upon exam:
▫️Hypermobile hamstrings
▫️Tenderness & tension at proximal hip flexors
▫️Hip abductor & extensor weakness bilaterally
▫️Limited posterior expansion of ribcage with breath & limited thoracic rotation
▫️Pelvic floor muscle tension
💡What did we do to help?
✅Lots of education on bladder function/norms/irritants, hydration, fiber/nutrition for constipation, how pelvic floor tension can contribute to symptoms of urgency or bladder pain, self management strategies to do at home if symptoms flare
✅Manual pelvic floor muscle release
✅Bladder mobilizations
✅Abdominal tissue work
✅Hip flexor, diaphragm & ribcage releases
✅Colon massage
✅Dry needling for nervous system calming
✅Specific exercise prescription to address any strength or mobility deficits we found
⭐️Her frequency of symptoms has reduced significantly, and intensity of “flares” when they happen have also significantly reduced. She has not needed any of the prescribed medications and symptoms continue to improve by addressing her pelvic floor muscle dysfunction. 🎉
*Some patients do need medical intervention at times, but pelvic floor therapy & education on bladder health is such an underrated place to start!
