Bloom & Be Therapy and Educational Services Bloom and Be provides assessment and intervention in the areas of speech-language therapy, reading disorders, and post-infant feeding.

Bloom is owned by a speech-language pathologist and homeschooling mom in Muncie, IN. We provide evaluations, therapy, caregiver training, IFSP/ISP/IEP paperwork review and advocacy, and other needed services for individuals with a variety of communication disorders, reading disorders, feeding disorders, etc. We provide therapy in both office and teletherapy settings but can occasionally accommodate other meeting areas when needs arise. Occasionally we offer homeschool classes! In the past, these classes have included American Sign Language and Executive Functioning. Therapy goals are customized to your family's needs. Our goal is to equip families to foster a communication supportive and independent environment. YOU are your child's most important model so we want to empower you to help your loved ones communicate and participate in their world in the most functional way possible.

8. Release of Information: This form is optional.

What this form does, is allows us to exchange information from other facilities. For example, if your child gets therapy elsewhere or you want us to communicate with their school. Without this form, we are now able to give out any of your information or ask for any from another place.

We only use this to help with your child's care.

The man I’m standing beside is the reason I was able to get a drivers license.

Long before I had a platform, before the book, before professional speaking…
in addition to a wonderful mother,
I also had an occupational therapist.

Something people don’t tend to think about is how services like OT and speech therapy are typically delivered just ONE hour a week.

I don’t care what you’re try to achieve, no one makes ✨life-changing✨ progress with just one hour a week.

If REAL therapeutic progress is what you’re after, that means whatever happens the rest of the week…
at home, in the car, at the dinner table, during homework,
is just as important as time spent in session.

Occupational therapy helped me get a drivers license…but my mom is largely to credit for my ability to pass that test.

Why?
Because she made me, a grumpy teenager at the time…do my damn therapy homework.

It should be noted, practicing therapy goals at home is often HARD on families. Parents are tired, siblings need attention, and work schedules are complicated.

But if you want your child to get the full benefit of therapy…as someone who RECEIVED therapy…

I urge you to ask your therapist what to practice. Ask them to show you how. Ask what it should look like at home.

And then find small ways to make it possible.

The families who make the biggest gains usually aren’t the ones with the most therapy hours. They’re the ones who turn everyday moments into practice opportunities.

Don’t hear what I didn’t say, you can’t, and shouldn’t turn EVERYTHING into a learning opportunity.

BUT, just know that therapy works best when it doesn’t STAY in the therapy room. 💜

7. Intake Form - This form IS required and it is so important and helpful! In order for us to do our best job evaluating each client, it is really helpful when we get background information from caregivers.

Not only do we want to know your concerns, but also what the client's STRENGTHS and LIKES are so we can engage them and bring in toys and tools that they enjoy!

6. Photo/Video Release: This form is NOT required. We have it as part of our forms because sometimes we record or photograph clients to help as part of their therapy (making social stories, increasing self-awareness, etc.). Occasionally (and with permission) we enjoy sharing photos of our clients on social media.

You are 100% welcome to NOT sign this form and we will never share your child's photograph or videos in any way.

5. Practice Policies: This is the meatiest document full of lots of info! Make sure you read the whole thing because it answers a lot of questions! Here are some highlights:

- caregivers may not leave the facility during their child's session
- we operate on the clinical hour so 30 minute sessions end at 25 minutes with 5 minutes left for homework, clean up, and transition
- breaking the attendance policy can lose your spot
- please pay bills within 5 days
- we can give superbills if needed
- please use Simple Practice messages instead of texting when possible

4. Privacy Practices: This states that we won't share your personal health information with anyone else. We follow HIPAA. This one is pretty self-explanatory. ;)

3. Consent to Treat: This is a common form when seeking any form of healthcare. This form is also a non-negotiable part of our paperwork. We need to be able to prove that you agreed to come in and let us evaluate and/or treat your child.

We are hopeful there would never be any issues that come up where we would NEED this, but it's better to be safe than sorry!

2. Credit Card authorization: We encourage having a credit card on file in order to get the prompt pay discount the clients get for paying their bill within 5 days. We do our best to make sure you understand what you will owe up front. When billing changes happen, we try to fix changes and/or alert you before new charges go through.

If on medicaid, you do not need to fill out this form.

1. Consent for telehealth - Even if your appointments are scheduled in-person, a time may come up where you would like to switch to virtual due to weather or another reason. It's helpful that we have this signed in advance.

Paperwork highlight!

Just a reminder that our "Prompt Pay Rate" is called that because invoices need to be paid within 5 days of being sent out.

Per our paperwork, after those 5 days you may be responsible for a greater balance per your insurance paperwork.

We may be getting rid of our prompt pay discount in the future, but until then we wanted to put out a clarifying reminder. Thanks!

Hello! I am a speech-language pathologist and homeschooling mom in Muncie, IN. I provide evaluations, therapy, caregiver training, IFSP/ISP/IEP paperwork review and advocacy, and other needed services for individuals with a variety of communication disorders. I provide both in person and teletherapy. Therapy goals are customized to your family's needs. I specialize in working with individuals who are non-verbal or minimally verbal but I have much experience in other areas as well. My goal is to equip families to foster a communication supportive environment. YOU are your child's most important model so I want to empower you to help your loved ones communicate in the most functional way possible.
