Parent or Guardian's ID
Child's ID (Birth Certificate, Social Security Card, Medicaid Card)
Person Needing Certified
Current Address
Proof of Income for the last 30 days for the entire household. WIC Folder

Supplemental Foods Available:
Milk, Cheese, Juice, Cereal, Whole Grains, Fruits and Vegetables, Eggs, and Beans or Peanut Butter

Free program for pregnant individuals to receive support and education!

When you are expecting a new baby it can be exciting, but it can also be scary and lonely at times. However you might be feeling about your pregnancy, surrounding yourself with other moms is one of the best things you can do for yourself.

Socialize and interact with local women in similar stages of pregnancy, and learn valuable information to prepare you for motherhood. Facilitated by a licensed mental health professional and Doctoral level student from the Ball State Counseling Practicum Clinic. This FREE program is made possible by the Delaware County Health Department.

As part of my annual newborn and maternity education retreat, I’m looking for a very small number of models to help me practice new techniques before offering them to regular clients.

I’m seeking:
• 2 maternity models
• 2 newborn models
• Sessions scheduled February 9–20th (weekdays)
• New-to-me clients only
• Not currently working with another photographer
