Spring Creek Post-Acute Rehabilitation offers a variety of services to our residents. We are a 226-bed long-term care, Medicare-certified facility which provides comprehensive rehabilitation and restorative services. Each room is cable and phone ready. We love our community and want to use social media to connect with you! Spring Creek Health Care offers a variety of services to our residents. We are a 226-bed long-term care, Medicare-certified facility which provides comprehensive rehabilitation and restorative services. We offer private and semi-private rooms. Each room is cable and phone ready.

Our success rate with our short term rehabilitation program has been extraordinary. The comprehensive care plans are designed to facilitate a quick and complete recovery. Plans are incorporated into the individual resident’s rehabilitation programs with the goal of their independence and the ability to return to community living. Spring Creek Health Care also emphasizes support and education for residents and their caregivers prior to returning home. We love our community and want to use social media to connect with you!
