I’ve had the most pleasurable summer of training and racing. Yesterday’s race didn’t go as planned. But the photos are so darn cute that I can’t even be mad about it.

I made a couple mistakes. I knew better than to spend 90 minutes shivering in a space blanket before the race. And I knew better than to wear a newly released pair of super shoes that I had yet to train in.

But it was still a great effort.

I worked my way through flight school, while being a single mom to Sparkle.

On Friday I got a window of good weather in the afternoon to get it done. I am very pleased with my checkride performance. The additional training made all the difference.

When I impulsively signed up for flight school 2 years ago, I had no idea what getting a license would entail. It required a level of endurance that I had to develop along the way. I did not have a well defined “why”. I was just seeking a different buzz from the monotony of sagittal plane endurance sports. Through the process, I discovered a realm that I never knew existed within, and around me.

I passed my FAA Knowledge Exam!!!

If I knew how much of a time investment flight school would be, I probably never would have started. But that can be said about anything.

I love what I am becoming through this process. And nothing can parallel the buzz I get from flying.

Achilles ruptures are injuries that I treat quite frequently in my clinic. Sometimes I have seen the patient previously for midsubstance or insertional Achilles tendonitis, and the next time they present, their vulnerable Achilles collagen has predisposed them to an acute rupture. Other times, the rupture is spontaneous--without predilection.

An acute Achilles rupture can be treated both conservatively, or surgically. I will ALWAYS surgically repair a ruptured Achilles, unless a patient is medically unstable. Meaning, the risk of anesthesia, or compromised skin healing, overrides the benefits of surgical repair.

I prefer to do an open Achilles tendon repair vs a percutaneous repair. I like to get a good look at the rupture, and irrigate the hematoma. I make my skin incision just medial to the Achilles centerline, to avoid damage to the sural nerve. I use 1.3mm SutureTape, and a Krackow locking stitch to secure both ends of the tendon. I then tie both ends together with the foot maximally plantarflex to obtain appropriate tendon tension. This takes about 10 min, and then an additional 20 min to carefully close the incision.

I don't augment my repairs with grafts, PRP, amnion ect. The subcutaneous layer supplies adequate blood flow to the tendon. Literature supports that Achilles repairs heal 100% of the time without augmentation.

Post-op recovery is the hardest part when dealing with Achilles ruptures. In a nutshell, full return to activity takes on average 8.2 months +/- 2.9 months. Physical therapy is essential to properly regain strength. Stretching and eccentric loading should be avoided for the first 3 months of rehab.

SHOCKWAVE THERAPY FOR INJURIES

Good news, my clinic offers both Focal and Radial Shockwave--and the results have been unprecedented.

I have had patients present to me assuming that all shockwave units are created equal. That is a fallacy, and there are objective indications and outcomes specific for High Energy Shockwave (Focal), and Low Energy Shockwave (Radial).

The premise behind both Focal and Radial Shockwave is that waves of energy can facilitate tissue healing, and disrupt chronic pain patterns.

When a patient presents with an injury that has failed months or years or conservative treatment, my goal is get that injured tissue (bone, tendon, fascia) into a renewed phase of healing.

Shockwave achieves this by breaking down disorganized collagen, conducting stem cell precursors to the injured tissue, and increasing blood flow.

Think of it this way: we want to give your injury a controlled inflammatory response, so that we can reinitiate a stalled healing situation. Further, the pain you feel when shockwave is being administered is also desensitizing the pain receptors within the injured tissue, so that the tissue can then progress to a phase that it can actually heal.

The awesome part about specializing in the lower extremity, is that I get to diagnose and treat all systems in this area of the body: vascular, bone, soft tissue, nerves, skin, nails. I also get to troubleshoot the systemic problems that have led to the lower extremity issue.
