Getting to care shouldn’t be a challenge and at Hackley Community Care, we’re here to help make it easier.
When one of our dental patients faced transportation barriers, our team stepped in. After connecting with a Community Health Worker, they were linked to transportation support through Pioneer Resources.
HCC offers transportation support to help patients get to the care they need, when they need it.

This is unfortunately not an April Fools’ joke.
Due to unexpected mechanical issues causing a loss of heat, our Integrated Health Center (IHC) at 376 E. Apple Ave. will close at 2:00 PM on Wednesday, April 1.
We anticipate reopening at 12:00 PM on Thursday, April 2.
Patients with appointments will be contacted directly with more information.
