Lake Woods Nursing & Rehabilitation Center

A skilled nursing facility that provides both short-term rehab. and long-term care including physical, occupational, speech, social and recreational therapy.

Conveniently located near US 31, Lake Woods is a newly remodeled skilled nursing facility that offers short-term rehabilitation rooms for those recovering from surgery or injury, as well as extended nursing care beds for those individuals needing hospice, medically complex, wellness or memory care services. In addition to excellent care, Lake Woods provides a memory care unit. Residents can take advantage of the many activities offered, from special outings to carefully planned on-site events. We recognize that meals are an important part of personal enjoyment and socialization and we’ve developed a culinary health program that focuses on residents’ preferences and dining enjoyment. Our goal is to provide the highest level of quality care while maintaining a home-like environment. We strive to make our beautiful facility feel like a home away from home for every patient and resident we serve.

For two years in a row you have won the Peplinski Group Preventionist Team of the Year for promoting and providing the highest percentage of preventive vaccines to our residents and staff throughout all of our Peplinski homes, exceeding the national average.
