Here at Pathways it is our hearts desire to make a difference in the lives of the participant's and families that we support. We are family owned and operated! Owners Jimmie and Charlene, with 30 years total experience, have supported many children in foster care, and special needs. After the untimely passing of our most special gift, Charlene's uncle who had down syndrome, Jimmie and Charlene wanted to do more to make a difference in the lives of others. In 2000 Pathways was formed! Although we are small in size we strive to make a BIG difference! MISSION:

Mission of Service is to provide tailored quality services and supports to the consumer, in the amount needed and time frame required. All activities of this agency will be seasoned with the goal of exceeding the participants expectation of quality services and provision. STAFF:

All staff are trained to give the best in quality care. Staff is trained in CPR & First Aid, Medication Administration, NCI, Seizure Management, OSHA, HIPPA, Blood Borne Pathogens, Clients Rights, and CAP Core Competences. All staff meet the state requirements for paraprofessional status and are supervised by a Qualified Professional. Staff members are cleared through healthcare registry, background checks, and NC Dept of Corrections. DETAILS OF SERVICES:

In Home Skill Building: Provides habilitation and skill building to enable the participant to acquire and maintain skills, which support more independence. In-Home Skill Building augments the family and natural supports of the participants and consists or array of services that are required to maintain and assist the participant to live in community settings. Residential Support: provides assistance with acquisition, rentention, or improvement in skills related to activities of daily living, such as personal frooming and cleanliness, bed making, household chores, eating and the preparation of food, and the social and adaptive skills necessary to enable the individual to reside in a non-institutional setting. This service is provided to individuals who live in a licensed community residential setting, foster homes, as well as unlicensed alternative family living homes that serve one adult. Personal Care Service: under the waiver include support, supervision and engaging participation with eating, bathing, dressing, personal hygiene and other activities of daily living. Support and engaging participant participation is non-habilitative and describes the flexibility of activities that may encourage the participant to maintain skills gained during active treatment and/or rehabilitation while also providing supervision for independent activities of the participant. This service may include preparation of meals, but does not include the cost of the meals themselves. Engaging participant in utilizing skills gained during active treatment and/or rehabilitation is key and may be provided outside of the individual’s residence. Community Networking: provides individualized day activities that support the participants definition of a meaningful day in an integrated community setting with persons who are not disabled. This service is provided separate and apart from the consumer's private residence, other residential living arrangements, and/or the home of the service provider. These services do not take place in licensed facilities and are intended to offer the participant the opportunity to develop meaningful community relationships with non-disabled individuals. Services are designed to promote maximum participation in community life while developing natural supports within integrated settings. Community Networking services enable the participant to increase or maintain their capacity for independence and develop social roles valued by non-disabled members of the community. As participants gain skills and increase community connections, service hours should fade; however a formal fading plan is not required. Respite Care: is a service that provides periodic relief for the family or primary caregiver as detailed in the Person Centered Plan.
