The landscape of payment for community-based palliative care is diverse and rapidly evolving. With a range of options, including Medicare Advantage contracts, emerging Medicaid benefits, private payer innovations, and clinical collaborations, there are new avenues for generating sustainable revenue. However, experiences with these reimbursement models have varied significantly across organizations. In this session, participants will gain valuable insights and advice from both payer and provider perspectives on navigating the complexities of reimbursement for community-based palliative care.

Today is . There are 63 million people in the U.S.—approximately one in four adults—who serve as caregivers, family, friends, or anyone who provides care to individuals living with chronic or serious illnesses, disabilities, or mental health challenges. Many provide sustained, high-intensity care, assisting with activities of daily living and complex medical tasks. As a result, their own risk of anxiety, depression, physical illness, and financial strain increases.

Providing better support for caregivers is essential—not only for the well-being of patients and families, but also for the outcomes and cost-effectiveness of health care organizations. CAPC’s new report, The Case for Caregiver Support: Better Outcomes for People and Organizations, outlines why caregiver support is a critical component of high-quality serious illness care and makes the case that health care organizations should establish formal caregiver support programs to make a difference in both patient outcomes and health care spending. The leader’s challenge is to demonstrate effective use of program resources while advocating for the best care for patients. This session addresses the complexities in balancing efficiency with the unique needs of highly complex patients.
