Diamond State Primary Care is a Family Practice for ages 18 and up. We are currently accepting new patients.

Our staff is committed to providing quality care for adults and seniors. As part of the Trinity Health Accountable Care Organization, we provide a resource for healthcare and health information. We offer preventative care, acute care, and chronic care management. Our mission is to be you and your family’s core medical resource and to help you navigate the healthcare system. We strive to make sure you get the care you need, when you need it! Diamond State Primary Care’s highest priority is to keep you healthy. We strive to provide exceptional medical care by taking the time to listen to our patients’ needs and concerns. Dr. Imelyn Lahey opened the doors to Diamond State Primary Care in January of 2021.
