This patient has faint palpable pulses, pipeline calcifications in the arteries in the foot. WIth poor micro circulation can lead to poor would healing. Often found in renal disease, diabetes or aging vessels. Management: control of blood sugar and kidney disease, foot and wound protection, vascular consultation, nutrition, exercise and cessation of smoking. Achilles tendon reconstruction surgery with lengthening by side to side anastomosis. status post iatrogenic shortened Achilles relenting. Patient was unable to walk properly in severe pain for over a year after original surgery. . After Achilles tendon rupture surgery the foot relative to the leg was left in an abnormal 30 degree shortened position. (equinus). This patient was unable to ambulate properly. She could compensate only with severe external rotation of the leg. This surgery corrected this iatrogenic post surgical position. Patient is doing well. DrTotten.com.
