Through outreach and referral, an individual experiencing long-term depression, substance use, and limited access to mental health care was connected to critical support services. Barriers included inadequate insurance coverage, lack of transportation, and living in a community with few accessible resources. With support, the individual was reconnected to urgent mental health care and ongoing therapy, including virtual sessions when technology access was limited. Wellness planning focused on realistic goals, coping strategies, and increasing independence through transportation options and basic needs access.
Over time, additional supports were put in place, including health insurance coverage with transportation benefits, care management, and housing assistance. As stability increased, the individual was able to resume in-person therapy and move into safe, stable housing. This work reflects how consistent support, practical problem-solving, and access to services can create meaningful progress toward recovery and overall wellness. 🌟
