✨ Medicare’s GUIDE Program offers powerful support for families caring for a loved one with dementia, and Visiting Angels Newton/Canton is proud to help guide families through every step.

Through GUIDE, eligible Medicare members receive ongoing help from PocketRN, a program that provides:
• A dedicated dementia trained nurse for regular check ins
• 24/7 virtual access to a care team
• Support with medications, symptoms and other chronic conditions
• Coaching for family caregivers
• Community resource guidance
• Care coordination with physicians and specialists

As a participating provider, Visiting Angels delivers the in-home care and respite hours included in GUIDE while PocketRN manages clinical support and education. 💙

✨ One of the most meaningful parts of Medicare’s GUIDE Program is the respite care benefit that gives family caregivers a needed break while their loved one remains safely supported at home.

Eligible Medicare members living with dementia may receive 72 hours of paid in home respite care each year, provided by our Visiting Angels team. These hours can be used in 4 hour increments and scheduled when caregivers need rest, time away, or a chance to recharge. 💙
