• Must possess a current NYS Professional Registered Nurse license

Responsibilities:
• Triages and assesses acutely ill patients for early identification of signs and symptoms to prevent complications and ensure timely intervention of acutely ill patients.
• Educates patient and family in health promotion and disease management using available resources.
• Works with patients, clinical team and providers to ensure optimal patient outcomes
• Works with patients to maximize their health capabilities
• Manages the triage flow for sick calls and chronic care management program
• Administers medication and provides treatment prescribed by provider
• Performs immunizations and injections
• The RN provides clinical leadership of staff while overseeing the scope of practice of LPN’s and Medical Assistants in the outpatient office setting.
• In collaboration with the medical providers, participates in and facilitates Performance Improvement activities, monitors PI data, and assists with the development of Performance Improvement plans.
