Transition House... Changing lives by creating pathways for mental wellness! Our mission statement seems so simple, yet the challenges people face through their recovery from serious mental illness can make what appears to be a simple tasks become extremely challenging. It truly takes a strong team of determined advocates working along side our People to make the seemingly impossible possible.
