Each October, Waterfall conducts a patient experience survey to gather feedback on the care provided at our clinics.
If you're a Waterfall patient, keep an eye out for a survey link via text, MyChart, or email — or feel free to request a paper copy at the front desk during your next visit.
The survey is open through October 31, and your input helps us continue improving the care we provide.

Join us for a Trick or Treat fundraiser on October 25th from 12:00 to 3:00 PM at Starfish Youth Therapy Center!

This is a free, inclusive, holiday event that all children and families are welcome to attend. Our team will be transforming our clinic space into a Halloween “neighborhood” where children can go door to door to trick or treat.
