"THE BEST MEDICINE IN THE WORST PLACES"

11/13/2025

What's the difference between those who thrive under pressure and those who panic? It comes down to mindset, mentality, and situational awareness. Be prepared. Stay frosty. Keep your head on a swivel. Stop the killing, then stop the dying.

Many thanks to Major Millee and SFC Stecz for providing a tour of their specialized CBRN first response facility. They were also instrumental in facilitating a productive meeting to discuss opportunities for ISTM to provide training in support of their Combat Medic recertification program (68W).

Witnessing a scenario unfold that mirrors a training exercise is chilling. Yesterday's tragedy at a Minneapolis Catholic school, where we teach active shooter response, hit hard. Someone knew this was going to happen. Why didn't they speak up?

A lot of hard work and preparation paid off this past weekend as we successfully launched our first Executive Protection–Tactical Medicine Response (EPTMR) course. It was an honor to present this vital training to such a motivated group of law enforcement and EP professionals. We’re already looking forward to our next course in January!

In a real encounter, knowing the difference between concealment and cover can decide who walks away.

Concealment hides you. Cover protects you.

A bookshelf, curtain, or drywall might block a silhouette, but it will not stop a round. Concrete, steel, or an engine block will.

The instinct to tell one from the other cannot come from theory. It comes from repetition and stress-tested training.

At ISTM, we teach first responders and civilians how to identify cover, move through it, and shield others while providing medical care under fire. The difference between knowing and reacting is what keeps people alive.

What was the first time you realized how limited “cover” can actually be in a real environment?

He didn't hesitate. He grabbed his IFAK and combat gauze, shoving it into the neck wound until the bleeding stopped. That instinct, that plan he'd already made, saved a life. Vigilance and preparedness matter.

Law enforcement always scans surroundings and plans ahead. While civilians shouldn't be hyper-vigilant, basic situational awareness—knowing exits, assessing the room—can bring comfort and safety in public spaces. It's about enjoying life while staying prepared.

The phrase “active shooter” first appeared in the 1990s as law enforcement searched for a way to describe a threat that moved faster than traditional tactics could handle.

Since then, the term has shaped how agencies train, how responders think, and how communities prepare. But while the language has stayed the same, the threat continues to evolve, and training has to evolve with it.

At ISTM, we study that evolution as a reminder that readiness is never finished. Every course we teach is built on the same foundation: precision, speed, and courage under pressure.

The term may have started decades ago, but the responsibility to stay prepared is as urgent as ever.

How have you seen response tactics or training change over the years?

If someone can carry a cell phone, they can carry a tourniquet. We want everyone to have these essential skills. You're already training yourself just by being aware of your surroundings. It's a good thing to do.

We’re here in Sacramento at the McClellan Park Tactical Village gearing up for our first-ever Executive Protection Tactical Medicine Response Course.

Our team has spent months building a course that merges executive protection skills with tactical medicine, following TECC (Tactical Emergency Casualty Care) guidelines.

This training is designed for law enforcement officers, security professionals, and civilians involved in executive or close protection.

💥 It’s going to be intense, informative, and mission-critical.
