ANATOMY OF THE CUBOID BONE & CUBOID REGION

The cuboid is a key lateral midfoot bone, positioned between the calcaneus posteriorly and the fourth and fifth metatarsals anteriorly. Medially, it articulates with the lateral cuneiform and occasionally the navicular, making it a central structural link between the hindfoot and forefoot on the lateral column of the foot.

Anatomically, the cuboid has a plantar groove that accommodates the peroneus longus tendon. This groove is a crucial feature, as the peroneus longus wraps under the cuboid before crossing to the medial foot, contributing significantly to transverse arch support and first-ray plantarflexion. Any alteration in cuboid position can therefore influence both lateral and medial foot mechanics.

The cuboid forms part of the calcaneocuboid joint, a component of the midtarsal (Chopart) joint. This joint is inherently stable due to its saddle-shaped articular surfaces and strong ligamentous support, including the long plantar ligament and short plantar (plantar calcaneocuboid) ligament, both of which are vital for maintaining the integrity of the lateral longitudinal arch.

From a neurovascular perspective, the cuboid region lies close to branches of the posterior tibial nerve and lateral plantar structures, explaining why cuboid dysfunction may produce diffuse lateral foot pain rather than sharply localized symptoms.

Overall, the cuboid acts as a keystone of the lateral column, balancing mobility and stability during stance and propulsion. Even subtle anatomical displacement or joint incongruency can significantly disrupt foot mechanics, leading to pain, altered gait, and compensatory overload elsewhere in the foot.
