Improve your shoulder health and longevity by bracing appropriately during your lifts. We always talk about reducing pain is step 1. Optimizing your movement is step 2.

This bracing strategy is also called “screwing in” your shoulder. It engages your rotator cuff and improves stability at the shoulder. Your rotator cuff’s job is to allow for normal joint movement and improve stability.

You can use a band to passively pull your arm forward, then screw the shoulder in AGAINST the band. Think of getting your elbow crease towards the ceiling.

Here’s 3 exercises that have MANY roles in rehab. When we talk about healthy knees, these are your 3:

1. Single leg step down: get the knee over your toe. Focus on knee stability and the control!
2. Side plank with hip abduction: should be an absolute staple for any active adult. We use this frequently in our runners and individuals with back pain
3. ISO lunge lifts: phenomenal way to load the quads. Maintain an upright posture so your back leg’s quads are working.

Sciatica is always a hot topic. Not all sciatica is treated the same. The same RULES apply, but the INTERVENTIONS are different.

Think about a house with “a leak”. Not all leaks are treated the same. The same rules apply, but different things are needed to fix each leak.

Here are 5 facts about your sciatica

1. Respect the tingles - don’t try to push through positions that produce and worsen the tingles. Modify your position
2. If you have symptoms that go past the knee, we know it’s a “larger leak”. Larger leaks are sometimes harder/longer to fix.
3. More back pain might be a good thing. If you have radiating pain, we want to move the pain TO the spine. In that process you may have more spinal pain.
4. Physical therapy is the FIRST step. People often go to urgent care or see their primary care doc. It wastes valuable time that you can be healing. IF the symptoms are so unbearable (or you have symptoms in #5), you may need your doc to prescribe meds or do an injection to calm things down. Typically though, the primary will refer to PT.
5. You should be concerned if you’re falling, having foot weakness, having bowel/bladder changes. Touch base with your physician (primary or a neurosurgeon) to discuss next steps. Sometimes a course of PT is still done during this process.

Our approach is simple: let’s find WHERE your pain is coming from, THEN we find your deficits that may be contributing to your pain.

Our client is a long distance runner, and she came in after seeing multiple providers, and underwent months of rehab with no real progress.

On her own, she was performing loaded interventions + speed interventions without having some of the minimum requirements to MAXIMIZE her function.

We found limitations in her ankle range of motion, knee mobility, hip strength, and single leg stability. We start addressing these deficits in a STEP-BY-STEP fashion, and she’s starting to see results 💪🏾. Sometimes people progress things too quickly, leading to continued pain.

Definitely not for everyone with knee pain, but here’s a strategy we sometimes use for individuals experiencing joint-related pain with bending the knee.

A towel behind the knee helps to create some space in the joint as you sit your hips back towards your feet. The strap helps to stabilize the towel + create a little more force with the mobilization.

Typically right after this, we recheck the painful movement (squatting or just bending the knee) to see what changes were made 🙏🏾

Our eval process is 1 on 1 with our docs for an hour. The MOST important part though is the first 20 minutes. We sit and chat with you about your symptoms, what you’re experiencing, what you’ve tried to treat it, your workout routine, etc. That actually gives us an 80% idea of what’s going on.

THEN we look at movement. Watching your movement helps us determine which areas to examine in detail; what areas may be tight/weak/hypermobile; and what movements potentially produce your pain.

This is an example for someone with hip pain. Our upper extremity or spinal evaluations look very different. Not pictured are manual joint assessments to the hip, knee, and ankle, some plank assessments, and our breakdown of what’s going on + the timeline of recovery.
