National Social Worker Month Spotlight 💙

For Tara Ridings, social work has been a journey shaped by experience, growth, and a deep commitment to supporting others.

Tara earned her Bachelor’s in Social Work with a minor in Psychology from Bridgewater State College. During her undergraduate program, she completed internships in Child Welfare and Protection at the Department of Social Services in Taunton, Massachusetts — an experience that left a lasting impact and led to a full-time role after graduation.

After relocating to Connecticut, Tara’s path took an unexpected turn into skilled nursing, where she spent over 20 years as a Director of Social Services. It was there that she found her calling — supporting patients and families navigating chronic illness and end-of-life decisions. What once felt intimidating became work she grew to deeply value.

In 2018, Tara transitioned into a role focused on Palliative and Hospice Care, where she continues to find fulfillment today. One of the most meaningful parts of her work is witnessing the moment when individuals and families truly understand what hospice is and the comfort it can provide.

National Social Worker Month Spotlight 💙

For Shavonne Moore, social work provides an opportunity to advocate for and support individuals through some of life’s most challenging moments.

Shavonne earned her undergraduate degree from Western Connecticut State University and her Master’s degree from Fordham University. She chose to pursue social work because of the profession’s diverse opportunities to guide, advocate for, and support individuals in meaningful ways.

She later transitioned into home care and hospice after recognizing the need for stronger advocacy, education, and resources for patients and their families. In this role, Shavonne remains committed to helping individuals navigate difficult life transitions with dignity, compassion, and support.

Outside of work, Shavonne enjoys spending time outdoors, traveling, and being with family, while also making time to prioritize self-care and personal well-being.

For Ali Reynolds, social work is about meeting people where they are and helping them move forward.

Ali earned her degree from Louisiana State University and was drawn to social work because it provides the opportunity to support individuals as they work toward their goals. One of the most rewarding parts of her role is the trust that patients and families place in her when they share their stories and invite her into their lives.

Ali is especially passionate about hospice care, where her work centers on helping patients feel comfortable and improving their quality of life. She values the meaningful time she is able to spend with patients and their families and considers it a true privilege to be welcomed during such a sensitive and personal time.

Outside of work, Ali is active in her church, and her Christian faith provides the strength and encouragement that inspires her to continue doing this important work.

National Social Worker Month Spotlight 💙

For Kelly Trolander, social work feels less like a career choice and more like a calling.

Kelly earned her undergraduate degree from the University of Minnesota and later attended Lindenwood University in Missouri. Throughout her career, she has worked in both community and hospital settings and has also volunteered in disaster relief efforts.

Kelly shares that she has always felt comfortable in difficult situations and believes that everyone faces challenges in life. The most rewarding part of being a social worker, she says, is the opportunity to walk side by side with the individuals she serves and support them on their personal journeys.

During her time working in the emergency room and intensive care unit, Kelly supported many patients who were critically ill. She often spent time sitting with patients and families, listening, and helping guide conversations about end-of-life care and hospice. Through those experiences, she witnessed firsthand the compassion and support hospice teams provide — and knew it was something she wanted to be part of.

Her journey ultimately led her to Constellation Health Services.

Outside of work, Kelly enjoys spending time with her husband and their three amazing sons, as well as their registered therapy dog, Rosie, whom she lovingly calls “my girl.” She also loves traveling and staying active through swimming, cycling, and hiking.

💙 National Social Worker Month Spotlight 💙

Helping seniors and their families navigate the healthcare system can be overwhelming without the right guidance. For Jennifer Nosalek-Kennedy, LSW, providing that support is what inspired her to pursue a career in social work.

With over 10 years of experience in the healthcare industry, Jennifer has seen firsthand how complex medical decisions and uncertain next steps can leave families feeling overwhelmed. Her goal is to help provide the education, support, and reassurance families need during some of the most difficult moments of their lives.

Jennifer is especially passionate about dementia care and education. She is a Certified Dementia Practitioner and will be providing presentations on behalf of the Alzheimer’s Association, along with educational sessions for referral sources, families, and patients.

Looking ahead, Jennifer plans to continue expanding her impact by pursuing geriatric certification and health insurance licensure, allowing her to further support seniors and their families as they navigate care decisions.

National Social Worker Month Spotlight 💙

For Sarah Elliott, the path to social work began with a simple act of helping others.

Sarah earned her graduate degree from Southern Connecticut State University, but her passion for supporting older adults started much earlier. As a young person, she helped run errands for residents at an independent living facility where her mother worked as a Registered Nurse. Those early experiences gave her a deep appreciation for the older population and ultimately inspired her to pursue social work and become a strong advocate for others.

For Sarah, the most rewarding part of her role is seeing the smiles and gratitude from patients, knowing she has made a positive difference in someone’s day — no matter how small.

Her connection to hospice care is also deeply personal. Both of Sarah’s parents received hospice services, and the compassion and support they received had a profound impact on her. After their passing, she felt a strong calling to work in hospice and is grateful to now have the opportunity to support patients and families during such an important time.

Outside of work, Sarah enjoys traveling and reading, and she loves making people laugh and sharing stories. She believes that laughter can be powerful medicine, especially for patients who may need a light moment during difficult moments.

National Social Worker Month Spotlight 💙

For Barbara Charles, the path to social work may have started earlier than she realized.

Years later, while rereading her pre-teen diary, Barbara discovered that at just 12 years old she had written that she wanted to become a social worker so she could help others.

After college, Barbara began her career at age 21 with the Department of Social Services, where she spent 35 years working across many departments, including Child and Adult Protective Services, Medicaid, and Home Care.

During an assignment at Holly Patterson Nursing Home, Barbara discovered how much she loved working with the elderly. That experience led her to pursue her Master of Social Work at Adelphi University.

After graduating, Barbara began working per diem in a Certified Home Health Agency, where she found great fulfillment in supporting individuals who are working to remain independent despite medical challenges. Providing emotional support and helping patients understand their long-term care needs has been especially meaningful to her.

Even after retiring from DSS, Barbara chose to continue her work in home care and remains passionate about helping patients stay safe and comfortable in their own homes.

Outside of work, Barbara enjoys traveling and active vacations that include hiking, biking, and kayaking. She has completed bicycle tours in 10 countries and 22 states and is looking forward to hiking in the Swiss Alps this summer.

National Social Worker Month Spotlight 💙

For Renee Hunter, hospice social work became deeply personal.

Renee earned her Master of Social Work from Boston College in 1996 and chose the profession because she genuinely enjoys helping others and making a positive difference in people’s lives. She has always been a people person, and she values being able to listen without judgment, offer support, and provide guidance during difficult times.

Renee shared that she never originally planned to become a hospice social worker. That changed after her family experienced the unimaginable loss of their daughter 18 years ago.

During her grief, she experienced the profound impact that compassionate professionals had on her own healing process. Through that experience, she knew that one day she wanted to honor her daughter by offering that same support to patients and families facing end-of-life care.

Today, Renee considers it both a responsibility and an honor to support patients and families during such intimate and vulnerable moments.

Outside of work, Renee enjoys spending time with family and friends, traveling, relaxing at camp, and staying active. She also loves reading, scrapbooking, and trying her hand at baking.

Constellation is a family-owned, family-centered organization that has remained true to our commitment to providing the best patient experience and the highest quality outcomes. We believe this is accomplished by ensuring that everyone we connect with feels valued, trusted, and heard.

We are part of a diverse network of post-acute providers that offer community-based home health, palliative care, hospice, private duty, school-based OT/PT/ABA, and in-home ABA therapy through our broad network of providers.

We also rely heavily on the expertise of our own talented team, who continue to look for ways to implement innovative technologies, processes, and solutions in order to improve patient experiences and satisfaction. The growth and success we have seen over the last 10 years reflects our commitment to our founding principles, as well as our ability to adapt to change while continuing to treat people like they are family.
