The Maine team representing Constellation at the Annual Fall Health Care Conference & Expo!

Morgan, Karen, Kendra, and Jennifer attended Maine’s largest healthcare event of the year — bringing together 350–400 professionals and 70+ exhibitors from across the state’s care communities.

The conference featured educational sessions focused on industry trends, innovation, and resilience, along with valuable opportunities to network, share best practices, and celebrate the dedication of healthcare professionals across Maine.

Our new Hospice & Palliative Care Liaison for Maine, Kendra, joined the fun at Marshwood Rehab’s Trunk or Treat to represent Constellation!

She even brought her creative touch — carving a pumpkin with our Constellation logo to show off some team spirit!

Last Sunday, our Massachusetts/New Hampshire team joined partners from Reservoir Center, DPH, and Right at Home for the Alzheimer’s Walk at Gillette Stadium. Together, we walked in remembrance of loved ones we’ve lost and in support of families impacted by Alzheimer’s. 💜

Kelly and Jennifer each wrote a message on a flower to honor their loved ones — a touching reminder of why we walk. It was a beautiful day of compassion, connection, and community (and yes, they even met Pat Patriot! 🏈)

Our amazing Connecticut Team had a wonderful time joining Milford Health and Rehabilitation Center for their Alzheimer’s Walk!

The non-profit organization Flower Again gifted Constellation with a carload of stunning flower arrangements for our hospice patients. Our volunteers, RNs, community liaisons, and marketing team joined together to hand-deliver each bouquet — spreading joy to patients in their homes and local facilities.

Each arrangement was thoughtfully tagged with the message:
“From past beauty to present delight; we hope you find joy in every petal.”

Special thanks to Jill Robey, Founder and Chair of Flower Again, and Cindy Kemp, Constellation’s Hospice Regional Volunteer Manager, for making moments like these possible.

Our Story

Constellation is a family-owned, family-centered organization that has remained true to our commitment to providing the best patient experience and the highest quality outcomes. We believe this is accomplished by ensuring that everyone we connect with feels valued, trusted, and heard.

We are part of a diverse network of post-acute providers that offer community-based home health, palliative care, hospice, private duty, school-based OT/PT/ABA, and in-home ABA therapy through our broad network of providers.

We also rely heavily on the expertise of our own talented team, who continue to look for ways to implement innovative technologies, processes, and solutions in order to improve patient experiences and satisfaction. The growth and success we have seen over the last 10 years reflects our commitment to our founding principles, as well as our ability to adapt to change while continuing to treat people like they are family.
