National Social Worker Month Spotlight šŸ’™

For Jibin Thomas, social work is rooted in connection, presence, and purpose.

Jibin earned both his Bachelor’s and Master’s degrees in Social Work from Georgia State University. His passion for the field began through his involvement in his church community, where he worked closely with youth and families.

He shares that the most rewarding part of his role is the time he gets to spend with patients, especially those without strong support systems. Being present during those moments is something he deeply values.

Jibin chose hospice care because it allows him to walk alongside patients and families during some of life’s most vulnerable and meaningful moments, offering comfort, support, and presence.

Originally from Kerala, India, Jibin now lives in Massachusetts with his wife and daughter. He enjoys spending time with his family, producing music, and staying grounded in his faith.

National Social Worker Month Spotlight šŸ’™

For Ashley Sylvester, social work is about helping others navigate life’s most difficult moments with compassion and support.

Ashley earned her degrees from UConn, Simmons University, and the University of Kentucky, and is passionate about supporting individuals and families both emotionally and practically — helping them access resources, understand complex systems, and feel less alone.

She is especially drawn to hospice care, where she focuses on dignity, comfort, and quality of life. Ashley values being present for patients and families during such an important time and helping guide them through difficult decisions with empathy and honesty.

Outside of work, Ashley enjoys spending time with her family, going on cruises, and staying active.

National Social Worker Month Spotlight šŸ’™

For Kaylin Ross, LCSW, social work is rooted in compassion, advocacy, and connection.

Kaylin completed her education at Virginia Commonwealth University and was inspired by the values her mother instilled in her — to speak up for others and lead with compassion. She was also encouraged by mentors who believed in her and supported her journey to becoming a social worker.

One of the most rewarding parts of her role is the opportunity to connect with people from all walks of life, especially the elderly population, whose wisdom and life stories she deeply values.

Kaylin chose hospice to challenge herself and to be the support for others that she didn’t always have during times of loss. Providing comfort and presence during such a vulnerable time is something she holds close to her heart.

Outside of work, Kaylin enjoys building LEGO sets, a creative way for her to unwind and recharge.

Our Story

Constellation is a family-owned, family-centered organization that has remained true to our commitment to providing the best patient experience and the highest quality outcomes. We believe this is accomplished by ensuring that everyone we connect with feels valued, trusted, and heard.

We are part of a diverse network of post-acute providers that offer community-based home health, palliative care, hospice, private duty, school-based OT/PT/ABA, and in-home ABA therapy through our broad network of providers.

We also rely heavily on the expertise of our own talented team, who continue to look for ways to implement innovative technologies, processes, and solutions in order to improve patient experiences and satisfaction. The growth and success we have seen over the last 10 years reflects our commitment to our founding principles, as well as our ability to adapt to change while continuing to treat people like they are family.
