Center for Hospice Care We have been the hospice of choice for over 12,500 families since 1985.

It's Hartford HealthCare at Home Center for Hospice Care's mission to provide quality care for people with life-limiting illnesses & support for their loved ones before & after death Center for Hospice Care is a local nonprofit organization that provides end-of-life care in the home and often in skilled nursing facilities, as well as bereavement support services after a loved one's death. We are the oldest and largest community-based regional hospice in Eastern Connecticut, and are state licensed, as well as Medicaid/Medicare certified. We serve the needs of patients of any age with life-limiting diseases, including heart-related conditions, ALS, Alzheimer's, cancer and other illnesses. In addition to hospice and palliative care, we provide bereavement support free of charge to anyone suffering the death of a loved one.
