I came in back in August very scared. For the first time in years I was told I had stage IV DIE in addition to a 12 cm cyst (which was actually much larger). I was met with much hesitation and resistance to downright refusal.

When I met Dr. Seckin for the first time he set me at ease with his confidence and great disposition. I almost felt like family from the start. I was in awe of how he achieves excellence in everything he does, from sailing to campaigning for sick women. I trusted you immediately and stopped at nothing to make sure you were my doctor.

The FDA’s decision to remove the black-box warning from Hormone Replacement Therapy ( ) marks an important shift in .

For years, that warning created hesitation around post-surgical hormonal support, even when patients stood to benefit. This update acknowledges that, when appropriately selected and monitored, HRT can help maintain longevity— supporting bone density, cardiovascular protection, cognitive stability, and overall metabolic well-being.

At the same time, patients with a history of should be clearly informed that HRT may occasionally trigger symptom flare-ups, particularly if any residual disease remains. That is why excision surgery must be complete without leaving disease behind.

When endometriosis patients are in menopause, this doesn’t eliminate HRT as an option; it simply reinforces the importance of individualized planning, careful formulation choices, and close follow-up.

Against this backdrop, the complexity of endometriosis surgery becomes even more apparent.

Treating this disease requires balancing two core priorities: removing lesions effectively and safeguarding the hormonal function that supports long-term health.

When the ovaries are involved, that balance becomes especially fragile. Endometrioma excision is one of the most technically demanding procedures in gynecologic surgery, and outcomes depend heavily on precision, experience, and meticulous surgical judgment.

The goal is not just to eradicate disease but to protect the ovarian tissue and blood supply that sustain hormonal rhythm and long-term vitality.

Too conservative an approach risks persistent disease; too aggressive an approach can accelerate ovarian aging and push patients toward premature menopause.

Navigating that narrow space requires deep expertise and careful decision-making at every step.

Taken together, these shifts redefine what comprehensive endometriosis care should prioritize today: not only effective disease removal, but also the preservation of hormonal integrity, the protection of long-term health, and a clear, evidence-based roadmap through both surgery and recovery.

The creation of The Endometriosis Foundation of America with Padma Lakshmi was the first step in building a movement for awareness, education, and advocacy. Now, the establishment of the Research Center at Cold Spring Harbor Laboratory, opened with the help of Alexis Joel marks the next chapter—one dedicated to uncovering the science behind this complex disease.

My mission has always been clear: to bring understanding, compassion, and real solutions to those who suffer in silence.

Sciatic endometriosis represents one of the most challenging and frequently overlooked forms of deep infiltrating .

In some cases, atypical vascularities—enlarged, displaced, or aberrant pelvic vessels—may compress the sciatic nerve or its surrounding plexus. Such vascular entrapment can clinically mimic sciatic endometriosis, leading to severe radiating leg pain, weakness, sensory disturbances, and gait impairment.

The deep sacral region, encompassing the sciatic, pudendal, and other major nerves along with critical vascular structures, often exhibits significant anatomical variations and vascular aberrations that contribute to compression and neuropathic pain.

In this case, we successfully explored the entire course of the sciatic nerve and potential compression sites using real-time nerve mapping method, achieving complete decompression, and excised endometriotic implants from the pelvis. Decompression surgery is an effective approach for restoring neural function and providing sudden symptomatic relief in these complex presentations.

Mastery of pelvic neuroanatomy and meticulous identification of structures are essential for precise surgery!

🌍 Honored to have given two talks at the Annual Congress in Istanbul, sharing insights on extreme sciatic and thoracic endometriosis and AI recognition in peritoneal endometriosis — advancing both surgical and technological frontiers in our field.
