In the facial trauma world, the gold standard for repair is an open reduction, internal fixation procedure.
This is where we access the broken bones of the face, put them back into anatomic position and hold them in place with plates and screws on the inside.

Sometimes, however, the bones are so fractured and in so many pieces “comminuted”, that it isn’t possible to fixate them with plates and screws. In these instances we use something called an external fixator, which allows us to hold the bones in place from the outside while they heal.

This unfortunate young lady suffered a self-inflicted gunshot wound causing a highly comminuted fracture of the central lower jaw (mandibular symphysis and parasymphysis) along with multiple fractures of the upper jaw (maxilla).

We were able to primarily repair the soft tissue injuries inside the mouth, wired her jaws shut (maxillary-mandibular fixation) and stabilized the mandible with an external fixator.

In 6-8 weeks we should hopefully be able to remove her hardware and have a stable upper and lower jaw. 🤞🏽
