*******Office Closure Reminder ***********
As a reminder, after careful consideration, we have made the difficult decision to permanently close our office at the end of this year.

Please be advised that there is only one remaining week for in-person appointments, as our office will be closed during the week of Thanksgiving. We understand the importance of continuity in your healthcare, and we are committed to providing support during this transition period.

Appointments in December will be limited to urgent cases. Consequently, our online scheduling system will be disabled. However, we will continue to manage established medications for patients seen within the past 6 months, and renewal referrals for patients seen within the past year until the office closes. For medication refills please be sure to contact your pharmacy. They can fax requests to 405-759-7022.

We strongly encourage you to establish care with a new primary care provider as soon as possible, especially for any new diagnoses or screenings you may require.

An email has been sent out today with a list of providers for anyone needing assistance with their search. If you are a current patient and did not get the email, DM us with your email address or call the office with that information at 405-378-2119.

📢❗️Humana Healthy Horizons Medicaid Plan (soonercare)members will need to establish care with their assigned P*P as soon as possible in order to avoid any lapse in care. The grace period for coverage through our office as an out of network provider ends tomorrow 10/01/2024. If you are unsure of your plan you may contact the Soonercare Helpline at 800-987-7767 for assistance.

After years of dedicated service, our office will be permanently closing. We wish Maria Smith DNP,APRN all the best in her well-deserved retirement! Please join us in expressing our gratitude for their years of care and compassion. Letters and emails will be sent to all patients detailing next steps. Please be sure to contact our office to update your contact information so you don’t miss out on any correspondence during our final days. We are grateful for the opportunity we’ve had to serve our community and touch so many lives.

Also, after hours voicemail is not for leaving requests for appointments please call during normal office hours for that and leave a message during that time if necessary. Cold and flu season is upon us! Let's be mindful that all appointment spots are very valuable at this time. Your no show appointment could have been a spot for a sick baby or another person to get to be seen, treated, and on the road to recovery.

No Show Policy Reminder:
-Any established patient who fails to show or cancels/reschedules an appointment and
has not contacted our office with at least 24 hours notice will be considered a No Show and charged a $25.00 fee.
-Any established patient who fails to show or cancels/reschedules an appointment with no 24 hour notice a second time will be charged a $50.00 fee.
-If a third No Show or cancellation/reschedule with no 24 hour notice should occur the patient may be dismissed from Ebenezer Healthcare Services.
- Any new patient who fails to show for their initial visit will not be rescheduled.
-The fee is charged to the patient, not the insurance company, and is due at the time of the patient’s next office visit.
