It utilizes a team-based care approach providing a personalized, comprehensive healthcare plan to the most at risk, those without access to quality healthcare, the under and uninsured. Those enrolled in our program have better health outcomes, less visits to the emergency room, while continuing to live in their own home often times without ever having to go into a nursing home.
