• You may need to follow up directly with your prior provider to ensure your records have been sent to us.
• We’ve seen delays in receiving records, so your follow-up can make a big difference.

Due to flooding, ongoing travel advisories, and road closures in the Houghton, NY area, our Houghton clinic will not be opened the remainder of today.

We will continue to re-assess the situation and update you accordingly.

As the only Community Health Center in Cattaraugus and Allegany Counties, we strive to provide high quality, universal primary care to all patients regardless of a patient's ability to pay.
