We’re excited to share that CPHHE has received a $100,000 Community CareGrant from Aflac to expand our work in Omaha!

This funding will support community health navigators providing education, prevention, and connections to care for individuals at high risk of chronic disease—helping reduce health disparities and improve lives.

Grateful for this partnership as we continue working toward health equity in our community 💙
