About the Role:
The principle responsibility of this role is to serve as the main Helios Care liaison to nursing home facilities in the service area. This position is responsible for offering information and
support to patients in skilled nursing facilities, including the needs for hospice or palliative care.
This person is responsible for providing Hospice RN Case Management, symptom management and discharge planning for patients in local skilled nursing facilities.

*Ability to manage multiple cases with accuracy and empathy.

*Knowledge and experience with acute care in an institutional setting.
