My Minimally Invasive Monday… Subtalar arthrodesis via MIS approach. Anterior and anterolateral working portal. Shannon burr to create space, wedge burr with fluoroscopic guidance to remove bone debris. Irrigate copiously to remove bone paste. +/- bone autograft (I have my tricks) and then fixation with screws of choice (.life CREED ortholucent screws here).

Postop protocol similar as open to target primary goal of successful fusion, but significantly less op**te use. Smaller incisions = smaller risks of infection and narcotic use… that’s a win in my book.

Traditional Achilles’ tendon debridement is one option for insertional Achilles tendonitis… however it has a long recovery and often results in good but sometimes unpredictable results with residual bursal swelling, Achilles thickening, and soreness.
Alternatively the Zadek dorsal closing wedge osteotomy is a good alternative to this. Can be done , has a shorter recovery, and does not violate the Achilles. Originally described in 1936 and has ample recent data showing good results.
Combining minimally invasive surgery with the .life screws allows patients to get back to life sooner. Ortholucent screws that have excellent strength and allow for improved osteotomy visualization… gotta love it!

One of the more interesting fractures I’ve seen in a while. Not necessarily a ‘complex’ injury but mainly because it just jumps out on the images. Rock climber, sustained fall, direct trauma to the lateral leg. Pain isolated to the fracture site. No other injuries.

Ok so this is ‘technically’ a stable fracture given that the ligaments around the ankle are un-injured…. But are we gonna treat this non-op? Probably not.

Happy Friday y’all… lets talk bunions. For those of you who follow me, you know I’m a big fan of minimally invasive bunions…. But the truth is, it’s not “one size fits all”. Every now and then there are other procedures to consider.

Here’s an example of a young adult who had undergone a chevron by an outside doctor. This subsequently failed fairly quickly after the index procedure. What are some reasons for this failure? Notice that the patient has a good component of metatarsus adductus. My co-fellow has done multiple studies on the recurrence rate with metatarsus adducuts… it’s real.

So what are the options now? There’s likely many reasonable options including a proximal osteotomy or Lapidus. In my hands, it’s a Lapidus. Will do a rotational technique combined with correcting angular deformity. In my humble opinion, an akin is almost always beneficial as it allows the ‘finishing touch’ to dial in the alignment.

And yes, I am looking forward to being able to do MIS Lapidus in the near future
