Need a health form completed for summer camp or the upcoming school year?

Email your form to frontoffice@ppadocs.com. Your child must have be up to date on well checks.

Fees are $10 for standard processing (4–7 business days) or $20 for rush processing (2–3 business days). We can charge the card on file, or you may call to provide a different card.

Completed forms can be emailed back to you, or you can pick them up in the office. Please call us with any questions!

For weight checks and shot-only appointments, we offer walk-in availability.

Hours are 9:00–11:30 AM and 1:30–4:00 PM Monday through Thursday, and 9:00–11:30 AM and 1:30–3:00 PM on Fridays.

Please call ahead the day of your visit so we can add your name to the schedule and help ensure a smoother check-in process.

We’re celebrating 18 incredible years of Dr. Filardi being part of our PPA family! 💙

When she’s not taking amazing care of our patients, you can find her embracing her love of all things Disney—she’s completed THREE Disney half marathons 🏃‍♀️ (including the Star Wars Dark Side, Walt Disney World, and Princess Run—she had her eye on that Cinderella medal 👑✨!).

A true Disney enthusiast, Dr. Filardi loves traveling to Walt Disney World, searching for hidden Mickeys 🐭, relaxing at the beach 🌊, and exploring new places.

Join us in wishing Dr. Filardi a magical birthday filled with joy, adventure, and maybe a few hidden Mickeys along the way! 🎂🎈

Spring is here, and it's time to dive into water fun! 🌊 But with swimming comes the important job of keeping our little ones safe. 🧸

For toddlers and kids:

Never leave them unsupervised around water—even for a second! A focused adult is a must.
Teach water safety rules early and make sure they follow them (no running, no diving!).
Always use life jackets on boats or when near deep water.
Don’t trust inflatable toys as safety devices!

It's never too early to teach your child to swim—many are ready by age 1, and all should learn by age 5! 🌟

Rachael has been a wonderful part of our front office/business office team at PPA since 1999. Fun fact—both she and her kids have been patients here!

For many years, Rachael dressed as Glenda for Halloween, delighting so many little ones… though some little girls were very disappointed when they asked to see Glenda’s shoes and found a plain pair of sneakers instead! 👟✨
