I’ve seen a lot of posts citing the John’s Hopkins study linking Tylenol and autism.

The main issue with the Johns Hopkins study and similar observational studies linking acetaminophen (Tylenol) use during pregnancy to autism is the presence of confounding factors, particularly familial and genetic confounding, which can create spurious associations. While initial population-based analyses have shown a small increased risk of autism in children exposed to acetaminophen during pregnancy (hazard ratio [HR] 1.05, 95% CI 1.02–1.08), the absolute risk difference is extremely small (0.09% higher at 10 years) and is likely not clinically meaningful.

Crucially, when sibling control analyses are performed—comparing siblings discordant for acetaminophen exposure—the association disappears (HR 0.98, 95% CI 0.93–1.04), indicating that shared familial factors, such as genetics or environmental influences, are responsible for the observed association in conventional models. This methodological approach is considered a robust way to control for unmeasured confounding in pharmacoepidemiology.
