Time is running out!

Apply and enroll in MNSure by December 15 to get health insurance coverage beginning on January 1. Open enrollment goes through January 15, 2023.

HealthFinders will be providing a MNSure walk-in clinic with bilingual navigators on December 15 at our Faribault HFC office (1415 Town Square Ln) to answer and assist with any last minute questions. No appointment necessary!

A provider at HealthFinders Collaborative connected a patient with one of our Community Health Workers (CHW’s), to help her set up a diabetes management plan. Struggling with ongoing fatigue, headaches, and a language barrier, the patient had trouble reading food labels, remembering to write down her blood glucose levels and attend her appointments. Often the patient would go a month without medication because she didn’t drive and had to rely on others for transportation.
Community Health Workers (CHWs) are frontline agents of change, helping to reduce health disparities in underserved communities. At HealthFinders, CHWs offer access to services, provide health education, support care delivery, and promote advocacy. While delivering a culturally appropriate health education and information, our CHWs also help people get the care they need, give informal counseling and guidance on health behaviors, advocate for individual and community health needs, and provide some direct services such as first aid, blood pressure, and glucose level screening. What is particularly unique in this model is the fact that our CHWs meet our clients in their own environment such as where they live, eat, play, work, and worship.

Hot Chocolate and sweet treats will also be available for our visitors that day!
