Monday kicks off the first day of the school based clinic! We will have providers at school on Mondays and Fridays from 8:30am-11:00am.

Don’t forget, in order for your child to use the school based clinic you must fill out a new registration form for this school year.

We can’t wait to serve OC students and staff this year🩺

We've distributed the updated registration packets to each OC school. Please keep in mind that completing the new packet is crucial for your student to access our services at school.

Here are a few things to know…

-The School-Based Clinic will start Monday, September 15th.

-We will be distributing registration packets within the next few weeks.

-Your child MUST have a new registration form filled out to be seen by one of Triad’s physicians at school.

-Make sure you follow us for a weekly schedule and information about our clinic🩺
