Mission At Work🌟
Extended Housing is dedicated to preventing and ending homelessness for individuals in Lake County living with serious mental illness. Emily and Tim who lead our PATH (Projects for Assistance in Transition from Homelessness) outreach team, play a key part of fulfilling our mission.

Their unwavering compassion and great networking skills link clients with shelter, emergency food, medical/mental health services and more.🤩

Our Homeless Outreach Impact
In just the last three months, our PATH Homeless Outreach team made a significant impact in Lake County, helping 10 households transition into permanent supportive housing. For many of these individuals, this transition proved life-altering as eight of the clients also secured stable employment.🏡
