School / Sports / Camp Forms

Does your child need a form completed for school or camp (physical forms, sports forms, medication or therapy authorizations, etc)? We need additional time to complete forms, especially during the summer months. Please submit your forms through our portal at least 2 weeks in advance. Thank you!

Measles is back in the news again after a recent outbreak in a Florida school. The virus is wildly contagious and young children are especially vulnerable. That’s why AAP recommends children receive the measles vaccine between 12 to 15 months of age. If your child isn’t old enough to be vaccinated, there are other ways to protect them if an outbreak occurs in your community. After a year like 2020, it's more important than ever to remember the positive things in our lives. Although there has been a tremendous amount of stress put upon us as a healthcare provider, we still have much to be thankful for. We are thankful for our health, first and foremost. We are thankful for all of the doctors, nurses, and support staff in medical facilities around the country working tirelessly to provide care during this time. We are thankful for teachers who have had to rapidly adapt to educating our children virtually instead of in-person as they have been trained to do for so long. We are thankful to grocery store workers, warehouse employees, truck drivers and everyone who has made sure to keep grocery stores stocked with food to feed our families. We are thankful for all of our wonderful patients who have given us the priviledge of caring for their children throughout the years and the trust you have given us to do so even in such uncertain times. We are thankful for our own families that support us through these stressful days and day-by-day changes as we work to navigate the new way of life and healthcare that COVID-19 has created.

We hope everyone has a safe and Happy Thanksgiving, however yours will be celebrated this year. Please take a moment today to find the things you are thankful for.
