**Sign-On Bonus Plan for HHA/PCA caregiver at Heka Hapi Homecare LLC**
**Eligibility Criteria:**
- All newly hired Home Health Aides (HHA) and Personal Care Assistants (PCA) are eligible for the sign-on bonus.
-Caregiver must complete a minimum of 90 days of continuous service with Heka Hapi Homecare LLC.
- During each 90-day period, the caregiver should not have any unscheduled call-outs or instances of unexcused absence.
**Bonus Breakdown:**
- Caregivers who meet the eligibility criteria will receive a sign-on bonus of $1000.
- The bonus will be paid out in installments every 90 days of continuous service and no call-outs.
- At the end of each successful 90-day period of continuous service and no call-outs, the caregiver will receive a portion of the total bonus amount ($250).
**Payment Schedule:**
- After 90 days: $250
- After 180 days: $250
- After 270 days: $250
- After 360 days: $250
**Note:**
- In the event that a caregiver has an unscheduled call-out during a 90-day period, their eligibility for the bonus payment for that period will be forfeited.
- The bonus is subject to all applicable taxes and deductions as per local regulations.
- This plan is subject to change or modification at the discretion of Heka Hapi Homecare LLC.
