Important Reminders for Your First Visit:
1. Please return your completed new patient paperwork at least one business day before your appointment. This allows us time to review your information and best prepare for your visit.
2. Please bring all medications and supplements in their original bottles to your first appointment. 💊
3. If you've seen a primary care physician, please bring any relevant records. If you have specialist records, please bring those as well. 📁
4. Be sure to include your name, email address, and mobile phone number in your message if you need a reply.

We offer both telehealth and in-person appointments. While we strongly encourage in-person visits for your first appointment. Telehealth is a great option if you cannot travel to the clinic. 💻🤝
