The Intake Coordinator is the first point of contact for our patients, families, and referral partners. This role is critical to ensuring smooth admissions and excellent communication.

Key Responsibilities:
• Answer incoming referral and patient calls
• Process new admissions from hospitals, case managers, and physicians
• Verify insurance eligibility and authorizations
• Accurately enter patient information into EMR
• Coordinate start-of-care with clinical staff
• Communicate clearly with families, providers, and internal teams
• Maintain organized intake logs and documentation

Ideal Candidate:
✔ Experience in healthcare or home health intake preferred
✔ Strong communication and customer service skills
✔ Organized, detail-oriented, and dependable

The QA Nurse plays a vital role in maintaining high clinical standards by reviewing documentation and OASIS assessments for accuracy, compliance, and quality.

Key Responsibilities:
• Review OASIS assessments prior to submission
• Perform QA audits of clinical notes and charts
• Ensure documentation meets Medicare and CMS guidelines
• Identify documentation gaps and provide feedback to field staff
• Support clinicians with best-practice documentation guidance
• Assist with audit readiness and quality improvement initiatives

Ideal Candidate:
✔ Active Illinois LPN or RN license
✔ Strong knowledge of home health regulations and OASIS
✔ Prior QA or chart review experience preferred
✔ Excellent attention to detail and clinical judgment
✔ Ability to work independently
✔ Bilingual candidates a plus
