Gibson Area Hospital/Ford County WIC Services GAH WIC Services provides the Maternal Child Health Programs such as WIC & Family Case Management W.I.C. In accordance with federal civil rights law and U.S.

is a supplemental nutrition program for:
-Pregnant women (through pregnancy and up to 6 weeks after the birth or after the pregnancy ends)
-Breastfeeding women (up to the infant’s 1st birthday)
-Non-breastfeeding postpartum women (up to 6 months after the birth of an infant or after the pregnancy ends)
-Infants (up to the 1st birthday)
-Children (up to age 5)
W.I.C. provides healthy foods, nutrition education, and referrals to other health, welfare, and social services. Foods that are provided can include the following: milk, cereal, beans, juice, eggs, peanut butter, cheese, whole wheat bread, brown rice, tortillas, oatmeal, fruits, and vegetables. also provides baby cereals and baby foods to infants as well as formula for infants who are not breastfed. In order to participate in this program thru Gibson Area Hospital & Health Services you must live in Illinois and meet the W.I.C. income guidelines which can be found at the following website: http://www.fns.usda.gov/wic/wic-income-eligibility-guidelines

Family Case Management
Family Case Management is a comprehensive statewide management program that helps income eligible pregnant women, infants, and children to obtain healthcare services and other assistance that they may need in order to have a healthy pregnancy and to promote the child’s healthy development. Clients that are enrolled in this program receive assistance in various areas including arranging primary healthcare services, receiving developmental screenings and referrals, lead screenings, and referrals to other health, welfare, and social services. In order to participate in this program thru Gibson Area Hospital & Health Services you must meet the income guidelines. For more information about these programs and to see if you may qualify for these services, please call (217) 579-1087. Department of Agriculture (USDA) civil rights regulations and policies, this institution is prohibited from discriminating on the basis of race, color, national origin, s*x (including gender identity and s*xual orientation), disability, age, or reprisal or retaliation for prior civil rights activity. Program information may be made available in languages other than English. Persons with disabilities who require alternative means of communication to obtain program information (e.g., Braille, large print, audiotape, American Sign Language), should contact the responsible state or local agency that administers the program or USDA’s TARGET Center at (202) 720-2600 (voice and TTY) or contact USDA through the Federal Relay Service at (800) 877-8339. To file a program discrimination complaint, a Complainant should complete a Form AD3027, USDA Program Discrimination Complaint Form which can be obtained online at: https://www.usda.gov/sites/default/files/documents/USDA-OASCR%20P-ComplaintForm-0508-0002-508-11-28-17Fax2Mail.pdf, from any USDA office, by calling (866) 632-9992, or by writing a letter addressed to USDA. The letter must contain the complainant’s name, address, telephone number, and a written description of the alleged discriminatory action in sufficient detail to inform the Assistant Secretary for Civil Rights (ASCR) about the nature and date of an alleged civil rights violation. The completed AD-3027 form or letter must be submitted to USDA by:
